What the client sees

What a client experiences when they open their consent link, step by step.

When a client opens their consent link, everything is on one page, top to bottom:

  1. Safety information — a physical address and an emergency contact (name + phone). Both are optional; they can decline either. If it's already on file, the page just says so.
  2. Their documents — the page heading is tailored to who's signing: a client signing for themselves sees "Review and sign your forms," and a guardian sees "Review and sign {minor}'s forms." Each document is its own card with its title and a one-line summary, and Read full document opens it in full. There's no scrolling requirement and no separate box or name for each document.
  3. One signature — a single checkbox that names every document, a single typed full legal name (the typed name is the signature), and the date, which Psy180 records automatically when they sign. Then one Sign and submit button. A guardian's checkbox says they're signing as the minor's parent or legal guardian.
  4. A confirmation — "All documents signed" — plus a confirmation email.

Psy180's packet covers the privacy-practices acknowledgment, telehealth consent, state notices and minor/guardian consent. Your own informed consent to treatment is a separate document you keep in your practice.

After signing: for 30 days, the same link opens the signed copies instead of a signing page. The signer enters the client's date of birth (a guardian enters the child's), then can View or Download each signed form. After 5 wrong dates of birth in an hour, the page asks the signer to try again in an hour — a pause, not the lock. After 10 wrong dates of birth in all, the link stops opening the signed copies, and a new link won't reopen them: download the signed copies from Settings → Agreements & Documents and share them yourself.

You can preview the signing page yourself by opening a consent link you've issued that is still waiting on a signature.
