What the forms are for

A plain explanation of the consent and privacy forms you're asked to sign.

The forms are the standard agreements your therapist asks you to sign before care begins. They usually include:

  • A privacy-practices acknowledgment — a notice describing how your information is protected.
  • Telehealth consent — for sessions held remotely (by video, phone, or secure messaging).
  • State notices — for states that have their own notices.
  • For a minor — a parent or guardian's authorization, or the minor's own consent where their state allows it.

These Psy180 forms don't include your therapist's own informed consent to treatment — that's a separate document your therapist provides.

Each form's full text is available on screen as you sign (open any card to read it in full), and that wording is the authoritative version — so read it before you agree. Psy180 is the secure platform your therapist uses; it doesn't provide the therapy itself. If anything is unclear, your therapist can explain what each form means for your care. (This is general information about the forms, not legal advice.)
