When you open your link, everything is on one page, and you sign once for all of it:
- Read — each form is a card you can open to read in full. You don't have to scroll to the end of each one.
- Check the one box — it lists every form you're agreeing to.
- Type your full legal name — your typed name is your signature.
- Press Sign and submit. The date is recorded automatically when you sign.
At the top of the page you may be asked for a physical address and an emergency contact — both are optional, and you can decline either. When you're done, you'll see "All documents signed" and receive a confirmation. For 30 days afterward, the same link lets you view or download your signed forms; it asks for your date of birth first (a parent or guardian enters the child's). If you enter the wrong date of birth 5 times in an hour, the page asks you to try again in an hour. After 10 wrong tries in all, the link stops opening your forms — ask your therapist for copies. Take your time, and ask your therapist anything before you sign.
